Every 15 tears, Isaac fires a barrage of 5 spectral cream tears, each dealing his damage. Each cream tear can have 1 of 4 effects.

1. Homes in on and pierces through enemies.
2. Bounces and splits into 4 tears on impact.
3. Wiggles and confuses hit enemies.
4. Increases in damage and knockback over time.
